• Application form for an operating permit for a catering establishment with information (in Dutch)
• Supervisor statement form (in Dutch)
• Notification form for depreciation of manager (in Dutch)

If the application is complete and there are no details, you will receive a decision on your application within 8 weeks after we have received your application. In case of special circumstances, the period for processing your application can be extended by 8 weeks.
