Slot machines

For this you need the Chamber of Commerce number of the organizing association.

You can apply for a permit for a lottery or report a small game of chance by means of a digital form, for which you must enter your Chamber of Commerce number. At slot machine halls you must also send other documents, make sure you have these available digitally: 

• A copy of the statement of conduct (not older than two months at the time of the application) 
• Evidence from an addiction care facility showing that business executives and administrators have sufficient knowledge and understanding of the use of slot machines and the associated risks of gambling addiction 
• Valid proof of identity of manager(s) (and a residence permit if the person does not have Dutch nationality)

You pay € 56,50 to process an application for one slot machine. For each subsequent slot machine you pay € 34,00. Even if you do not receive the permit, you will still have to pay for the application to be processed.
