Municipality Heemskerk: Certificate of conduct for individuals (VOG)
If you apply for a job, you may be asked for a Certificate of Good Conduct (VOG). This happens if the position involves working with confidential data, such as data about money, property or vulnerable people.
Information and fees for application for VOGApplying for a Certificate of Good Conduct (VOG) is done at the municipality where you are registered in the basic registration of persons. For this you bring along:
- Your valid identification (passport, driver’s license, Dutch identity card or residence permit).
- A fully completed and signed application form.
Minors up to 18 years of age
- Children from 16 to 18 years who apply for a VOG certificate do not need to have permission from their parent or legal guardian. They can submit the VOG application themselves at the counter.
- Children younger than 16 years of age must bring permission from parent/guardian. (This is a letter with signature/copy of proof of identity. Parents can also come with their child).
You will receive the application form from the person who needs a VOG from you. This may be an employer, an association or a government agency. The form indicates the purpose of the application. You can also authorize someone to apply for the certificate for you.

Your employee or volunteer pays for the VOG application. The cost of a VOG can be found on the municipality’s website.
